The Clinical Assessment Advisor plays a pivotal role in ensuring the optimal clinical outcomes for patients in home health care. This role involves collaborating with physicians and other healthcare professionals to develop and implement personalized care plans that cater to the unique needs of each patient. The Clinical Assessment Advisor is responsible for conducting thorough assessments, identifying areas of improvement, and coordinating with the care team to ensure seamless delivery of services.


Key Responsibilities:

  • Conduct comprehensive patient assessments to identify areas of improvement and develop targeted care plans
  • Collaborate with physicians and other healthcare professionals to ensure accurate and timely communication of patient information
  • Coordinate with the care team to ensure seamless delivery of services and optimal patient outcomes
  • Monitor patient progress and adjust care plans as needed to ensure continuous improvement
  • Participate in quality improvement initiatives to enhance patient care and outcomes


Join Our Team of Dedicated Professionals

Requirements:

  • Bachelor of Science degree in Nursing (BSN) or equivalent experience
  • At least 2 years of clinical experience in home health care, with a minimum of 1 year in a leadership or management role
  • Current CPR certification
  • Excellent communication and interpersonal skills
  • Knowledge of state and federal regulations governing home health care
  • Ability to work independently and as part of a team
