Clinical Assessment Advisor

We are seeking a highly skilled and experienced Registered Nurse to join our team as a Clinical Assessment Advisor - Home Health. As a key member of our care team, you will play a vital role in ensuring the highest quality of care for our patients.
Key Responsibilities- Conduct Comprehensive Assessments: Perform initial and ongoing evaluations of patient needs, identifying areas for improvement and developing personalized care plans.
- Lead Care Teams: Coordinate and manage care teams, ensuring seamless communication and collaboration among healthcare professionals.
- Develop and Implement Care Plans: Create and implement care plans that address patient needs, promote health and wellness, and prevent hospital readmissions.
- Monitor and Evaluate Patient Progress: Regularly assess patient progress, making adjustments to care plans as needed to ensure optimal outcomes.
- Collaborate with Healthcare Professionals: Work closely with physicians, therapists, and other healthcare professionals to ensure comprehensive and coordinated care.
- Ensure Regulatory Compliance: Maintain knowledge of and adhere to relevant laws, regulations, and industry standards, ensuring compliance and minimizing risk.
- Education: Bachelor of Science degree in Nursing (BSN) preferred.
- Experience: At least 2 years of clinical home care experience, with a minimum of 1 year in a clinical management role.
- Certifications: Current CPR certification.
- Skills: Excellent verbal and written communication skills, excellent interpersonal skills, and knowledge of state and federal home health agency regulations and compliance standards.
