Job Title: Clinical Assessment Advisor - Home Health

Join CenterWell Home Health, a leading provider of home health services, as a Clinical Assessment Advisor. In this role, you will play a critical part in delivering high-quality patient care and ensuring optimal clinical outcomes.

Key Responsibilities:
  • Conduct initial and ongoing evaluations of patient needs, developing and implementing personalized care plans.
  • Collaborate with healthcare professionals to ensure seamless care coordination and effective communication.
  • Monitor patient progress, making adjustments to care plans as needed to achieve optimal outcomes.
  • Participate in special projects and contribute to the growth and development of our team.
Requirements:
  • Bachelor of Science degree in Nursing (BSN) preferred.
  • At least 2 years of clinical home care experience, with a minimum of 1 year in a clinical management role.
  • Current CPR certification.
  • Excellent verbal and written communication skills.
  • Knowledge of state and federal home health agency regulations and compliance standards.
