Job Summary

CenterWell Home Health is seeking a skilled Clinical Assessment Advisor to join our team. As a key member of our care team, you will play a vital role in ensuring our patients receive the highest quality care in the comfort of their own homes.

Key Responsibilities:
  • Conduct comprehensive assessments of patients' needs and develop personalized care plans to achieve optimal clinical outcomes.
  • Collaborate with healthcare professionals, including physicians and other licensed clinicians, to ensure seamless care coordination and effective communication.
  • Oversee the development and implementation of care plans, ensuring they align with patients' goals and needs.
  • Monitor patients' progress, making adjustments to care plans as necessary to ensure optimal outcomes.
  • Coordinate with interdisciplinary teams to ensure timely and effective care delivery.
  • Stay up-to-date with industry developments, best practices, and regulatory requirements to maintain expertise and provide high-quality care.
Requirements:
  • Bachelor of Science degree in Nursing (BSN) preferred.
  • At least 2 years of clinical home care experience, with a minimum of 1 year in a clinical management role.
  • Current CPR certification.
  • Excellent verbal and written communication skills.
  • Knowledge of state and federal home health agency regulations and compliance standards.
  • Ability to work independently and as part of a team.
