Job Title: Medical Social Worker

At PeaceHealth, we strive to provide exceptional care to every patient, every time. As a Medical Social Worker, you will be part of a collaborative team working together to ensure our patients and their families receive the support and resources they need from diagnosis to discharge.

Job Summary:

We are seeking a skilled Medical Social Worker to join our Care Management team in a part-time position. This role will involve working with patients and families to identify and address their social and emotional needs, and coordinating discharge planning services to ensure a smooth transition to post-acute care.

Responsibilities:
  • Screen and identify patients who require care management services
  • Assess, develop, and implement comprehensive discharge plans in collaboration with the interdisciplinary team
  • Coordinate and facilitate improved ability to comply with treatment plans, counseling or support needed to cope with situations, and improved access to appropriate levels of care
  • Identify, plan, and arrange for appropriate services, applying knowledge of community resources, state, and federal health regulations
  • Actively support measures that promote effective use of resources
Requirements:
  • Master's degree in social work (MSW) or a related field
  • Washington State credential in social work, either Agency Affiliated Counselor Registration or a Social Worker, Therapist, or Counselor license
  • Demonstrated knowledge of community health, welfare, and social agencies
  • Excellent verbal and written communication skills, including sensitivity to other cultures and ethnicities
  • Proficient computer skills, including MS Office applications and electronic medical records
