Medical Social Worker

We are seeking a skilled Medical Social Worker to join our Care Management team at Southwest Medical Center in Vancouver, Washington. This part-time position will work 8-hour day shifts on Saturdays and Sundays, with an additional shift during the week.
Job Summary:The Medical Social Worker will be responsible for identifying and interacting with medically and psychosocially complex patients and families who require care management services. This includes coordination of discharge planning, assessment of patient needs, and development of comprehensive care plans.
Key Responsibilities:- Screen and identify patients who require care management services
- Assess patient needs and develop comprehensive care plans
- Coordinate discharge planning services with the interdisciplinary team
- Provide psychosocial support to patients and families
- Collaborate with community resources to ensure seamless care transitions
- Master's degree in social work (MSW) or related field
- Washington State credential in social work, counseling, or therapy
- Minimum of two years' experience in healthcare or social services
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
