Job Summary

We are seeking a skilled Medical Social Worker to join our team at PeaceHealth. As a Medical Social Worker, you will play a critical role in ensuring our patients and their families receive the support and resources they need during their healthcare journey.

Key Responsibilities
  • Identify and interact with patients and families who require care management and coordination of discharge planning services.
  • Assess, develop, and implement comprehensive discharge plans of care through an interdisciplinary team process.
  • Collaborate with the care team to identify problems or needs that require special planning, intervention, teaching, or follow-up.
  • Coordinate and facilitate improved ability to comply with the plan of treatment, counseling or support needed to cope with the situation, and improved ability to access appropriate levels of care due to lack of financial resources or lack of available services.
  • Actively support measures that promote effective use of resources.
Requirements
  • Master's degree in social work (MSW) or a related field.
  • Washington State credential in order to practice, either Washington State Agency Affiliated Counselor Registration or one of the other applicable Social Worker, Therapist, or Counselor licenses.
  • Demonstrated knowledge of community health, welfare, and social agencies.
  • Demonstrated knowledge of and ability to apply age-specific principles of growth and development and life stages to meet each patient's needs.
  • Demonstrated proficiency in social work practice.
Working Conditions

This is a part-time position working 8-hour day shifts, every Saturday, Sunday, and Monday. The ideal candidate will have excellent verbal and written communication skills, including sensitivity to other cultures and ethnicities. They will also have excellent skills in conceptual thinking, listening, problem resolution, and planning.
