Position Overview:

We are seeking a dedicated Registered Nurse to join our dynamic team within the Nutrition and Gastroenterology Clinic. This role is ideal for an individual who thrives in a fast-paced healthcare environment.

In this role, you will be responsible for delivering both direct and telephonic nursing care, engaging in health education, advocating for patients, and overseeing the coordination of care provided by other healthcare professionals. Your approach will be tailored to ensure it is appropriate for various age groups and developmental stages.

Key Responsibilities:

  • Develop and regularly update personalized care plans for patients seeking assistance with health promotion, disease management, or health-related challenges.
  • Prioritize and implement care interventions in collaboration with patients, families, caregivers, and the healthcare team, ensuring timely execution.
  • Conduct evaluations and follow-ups on care provided, adapting to the evolving needs and conditions of patients.
  • Incorporate the understanding and involvement of patients' families and caregivers in the care plan.
  • Utilize ongoing assessment data to adjust diagnoses, expected outcomes, and care plans as necessary.
  • Exhibit strong patient advocacy through responsive actions to patient care concerns and teamwork with healthcare professionals.

Requirements:

  • Bachelor's Degree in Nursing or an Associate's Degree in Nursing with current enrollment in a BSN program.
  • Current licensure with the Texas Board of Nursing or Nurse Licensure Compact.
  • A minimum of one year of professional nursing experience.
  • Experience with EPIC software is highly preferred.
