RN (Per Diem/On Call) - Hospice Care Specialist

We are seeking a compassionate and skilled RN to join our Hospice team as a Per Diem/On Call RN. As a member of our interdisciplinary team, you will provide holistic care to terminally ill patients and their families, assessing their needs and developing individualized plans of care.
Key Responsibilities- Provide direct patient care, including assessments, medication management, and wound care
- Develop and implement individualized plans of care in collaboration with the interdisciplinary team
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions
- Participate in ongoing education and training to stay current on best practices and evidence-based care
- Current Washington State RN license
- 1 year of nursing experience, with some pediatric experience preferred
- Ability to work independently and as part of a team
- Excellent communication and interpersonal skills
