RN (Per Diem/On Call) - Hospice Care Specialist

We are seeking a skilled RN Nurse Clinician to join our Hospice Care Team in a per diem capacity. As a member of our interdisciplinary team, you will provide holistic care to terminally ill patients and their families, assessing their needs and developing individualized plans of care.
Key Responsibilities- Assess patient needs and develop comprehensive plans of care
- Collaborate with the IDG to anticipate changes in patient condition and promote patient and family choice
- Provide continuity of care and enhance the quality of life for dying patients
- Work collaboratively with the hospice team to ensure seamless care transitions
- Associate's or Bachelor's Degree in Nursing
- Washington Registered Nurse License
- 1 year of nursing experience, with some pediatric experience preferred
