Hospice Nurse Clinician

We are seeking a skilled and compassionate RN Nurse Clinician to join our interdisciplinary hospice team. As a key member of our care team, you will provide holistic care to terminally ill patients and their families, assessing their needs and developing personalized plans of care.
Key Responsibilities- Assess patient needs and develop comprehensive plans of care
- Collaborate with the interdisciplinary team to coordinate care and ensure continuity
- Provide emotional support and education to patients and families
- Work with patients and families to anticipate and prevent crises
- Ensure patient and family choice and promote quality of life
- Associate's or Bachelor's Degree in Nursing
- Washington Registered Nurse License
- 1 year of nursing experience, with some pediatric experience required
- Prior hospice, home care, or long-term care experience preferred
