Kintegra Health is seeking a dedicated Community Resource Advocate to enhance our outreach efforts. Reporting directly to the Community Outreach and Advocacy Manager, this role is pivotal in assisting patients with their healthcare requirements. Responsibilities include facilitating enrollment in pharmaceutical assistance programs, Medicare, and ACA Health Insurance, as well as guiding patients through the disability application process via the SOAR initiative.
Key Responsibilities:
- Deliver exceptional service to community members, patients, and agency staff.
- Utilize motivational interviewing techniques to identify the needs and resources of individuals.
- Educate clients on program requirements and assist with enrollment or referrals.
- Ensure compliance with MAP, Medicare, disability, ACA insurance, and community agency guidelines.
- Engage in quality improvement initiatives to enhance operational efficiency.
- Collaborate with staff to maintain a seamless workflow.
- Leverage data systems for effective communication and patient processing.
- Adhere to training and certification requirements for various community programs.
- Recruit volunteers to expand service capacity.
- Document activities accurately as required by program leadership.
- Foster positive relationships with community members and healthcare providers.
- Conduct outreach to strengthen community engagement.
Qualifications:
Minimum qualifications include experience in a medical office environment, with a preference for bilingual candidates (English/Spanish). Candidates should possess strong verbal communication skills, organizational abilities, and critical thinking capabilities. A High School Diploma or GED is required, while a Bachelor's Degree is preferred. Relevant certifications such as SHIIP Counselor, SOAR Counselor, and ACA Navigator are advantageous.
