Primary Care Opportunity

OSF HealthCare is seeking an Advanced Practice Provider to join our established Primary Care practice. As a key member of our team, you will perform patient assessment and assign diagnosis based on recorded assessment data and current scientific research. You will establish a plan of care and implement interventions in accordance with care paths, protocols, standards of care, and current research. Additionally, you will provide education and counseling to patients, families, and groups. The clinic hours are Monday through Friday, 8am-5pm, with a shift of three days a week. This role offers excellent work-life balance with no call or weekends.

Qualifications
Master's degree or higher in nursing or physician assistant studies from an accredited program
Current Illinois license as a nurse practitioner or physician assistant
Certification by the American Nurses Credentialing Center (ANCC) or the National Commission on Certification of Physician Assistants (NCCPA)
Minimum of two years of experience as an APP in primary care or a related field is preferred
Current Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certifications
Proficiency in electronic medical record (EMR) systems
