Medical Case Manager with Expertise in Rehabilitation Services

QTC Health Services is a leading provider of medical services, and we are currently seeking a skilled Medical Case Manager to join our team. This individual will be responsible for providing case management services for injured, ill, or recovering clients.
About the Role
This position requires a highly experienced Case Manager who can assess client needs and develop effective plans to meet those needs. The ideal candidate will have a strong understanding of rehabilitative strategies and experience working with government contractors or as a staff member. Additionally, they should possess excellent communication skills, both verbal and written, and be able to work effectively with a team.
Responsibilities
The successful candidate will be responsible for:
- Providing case management services for injured, ill, or recovering clients.
- Assessing client needs and developing effective plans to meet those needs.
- Coordinating medical services, including durable medical equipment, physical therapy, occupational and rehabilitative services.
- Utilizing electronic medical records and telemedicine technologies on behalf of clients and their family members.
- Communicating effectively with team members and community providers.
- Formulating timely case reports, including presentation of factual information and analysis of such.
Requirements
To be considered for this role, candidates must have:
- A Bachelor's degree in Nursing or a medically related field.
- A minimum of 5 years' experience in case management, preferably in military treatment facilities, home health care, and/or mental health facilities.
- Experience with care delivery, psychosocial support, and reimbursement systems.
- An understanding of rehabilitative strategies.
- Excellent communication and organizational skills.
Preferred Qualifications
Candidates who possess a Certified Case Manager (CCM) certification from the Commission for Case Management Certification or previous experience working with government contractors or as a staff member will be given preference.
Compensation
We offer a competitive salary range of $54-$59 per hour, with the possibility of an additional 8% bonus and annual merit increases up to 5%. Our benefits package includes full benefits, including medical, dental, vision, 401K, PTO, and more.
