Medical Care Coordinator for Individuals with Developmental Disabilities

We are seeking a skilled Medical Care Coordinator to join our team at ResCare Community Living. As a Licensed Practical Nurse, you will provide medical care and support to individuals with developmental disabilities in a residential setting. This is a rewarding opportunity to make a positive impact on the lives of our clients and their families.
**Responsibilities:**
* Provide routine medical care and observations to ensure the health and well-being of our clients
* Work closely with Registered Nurses and physicians to develop and implement individualized care plans
* Communicate effectively with clients, families, and healthcare professionals to ensure seamless care coordination
* Administer medications accurately and document client responses and side effects
* Monitor vital signs, weights, and wound measurements to identify trends and report changes to the care team
* Ensure compliance with state and federal regulations regarding medication administration and documentation
* Participate in interdisciplinary team meetings to share knowledge and best practices
* Collaborate with home/facility/pharmacy staff to consult on psychotropic medications, side effects, and medication reviews
* Order, review, monitor, and discontinue medications as ordered by physician
* Document all interactions, including communication with clients, families, and healthcare professionals
* Maintain accurate and up-to-date records of client care and services provided
* Participate in on-call duties to ensure weekend and after-hours access to clinicians by staff and individuals served
* Participate in clinics completed at the operation with PCP, psych, etc. as assigned
* Work hours may vary - weekends and evening shifts/visits may be required
**Requirements:**
* Completion of a state-approved training program (e.g., diploma, associate's degree, etc.) per state requirement
* Must be a Licensed Practical/Vocational Nurse (LPN or LVN, per state licensure requirements) in good standing
* CPR certification
* At least 6 months of direct nursing experience
* Experience in providing services and supports to individuals with developmental disabilities, or related disorders preferred
* Valid driver's license, with a satisfactory driving record, as defined by ResCare's vehicle policy
* Ability to communicate verbally and written with all levels of personnel, internal and external to the company
