About the Role:

The Stark County Board of Developmental Disabilities seeks a skilled Assessment Specialist to manage Home and Community Based Service (HCBS) waivers. As an Assessment Specialist, you will conduct assessments, identify alternative resources, and collaborate with other agencies to ensure timely delivery of services.

Responsibilities:
  • Conduct thorough assessments for immediate need and current need following established guidelines and timelines.
  • Enter assessment results into the state system and maintain accurate records.
  • Collaborate with other SSAs and outside agencies to obtain necessary documentation and information.
  • Work closely with the waiting list team to administer HCBS waivers efficiently.
  • Attend regular meetings to review assessment outcomes and discuss areas for improvement.
  • Develop and implement strategies to ensure timely follow-up on completed assessments.
  • Provide exceptional customer service to support the board's mission and vision.
Requirements:
  • Bachelor's degree in a related field or equivalent experience.
  • Valid Ohio Driver's License and acceptable annual BMV Check.
  • DODD Certification eligibility is required.
  • Experience working with individuals who have developmental disabilities is preferred.
