Overview

This role offers a unique opportunity to provide personalized, evidence-based medical care to seniors and patients with complex needs in various residential settings.

Job Summary

The House Calls Nurse Practitioner (FNP/AGNP) provides comprehensive patient care plans, examining and treating chronic and urgent illnesses, collaborating with healthcare professionals, and ensuring quality healthcare delivery.

Responsibilities

  • Provides evidence-based medical care by planning, directing, and evaluating patient medical needs
  • Examines and treats chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses
  • Collaborates with caregivers and other healthcare professionals in providing interdisciplinary care
  • Maintains comprehensive patient care plans and ensures patient medical records are up-to-date

Qualifications

  • Active state Nurse Practitioners license
  • Current Nurse Practitioner National Board Certification
  • Active state DEA licensure
  • Master's degree in Nursing (MSN)
  • Five or more years of nursing experience; at least two years of nurse practitioner experience preferred
