Key Responsibilities:- Perform intake, assessment, and triage of patients referred to the Prostate CyberKnife Program
- Serve as a liaison between referring physicians and our team, providing information and access to the system
- Screen patients for relevant information and manage the acquisition of pertinent medical records
- Plan and implement the patient evaluation process, facilitating the evaluation of patients by interfacing with referring physicians and services
- Coordinate the complex management of patients at various stages of their diagnosis, treatment, and follow-up, collaborating with all disciplines involved in the patient's care
- Identify opportunities for patient participation in clinical trials and communicate information to physicians, research coordinators, and referring physicians to promote clinical trial enrollment
- Navigate patients and caregivers through the continuum of care, providing education and resources about medical conditions, diagnostic procedures, treatments, clinical trials, support services, and hospital processes
- Communicate patient progress and disposition to healthcare providers and maintain outcome data bases
- Manage IRB communications and regulatory updates, and have a working knowledge and access to multiple electronic systems to facilitate patient care and documentation
