Job Summary:

As an Ambulatory Care Pharmacist at Kaiser Permanente, you will play a critical role in providing high-quality, evidence-based pharmaceutical care services to patients. You will work collaboratively with other healthcare professionals to manage patient drug therapy, provide patient education, and prepare and/or oversee the preparation and dispensing of medications.

Key Responsibilities:
  • Conduct patient medication history inquiries and synthesize treatment plans, including drug therapy and lifestyle modifications.
  • Identify, resolve, and prevent potential and actual medication-related problems, including untreated indications, improper drug selection, and adverse drug reactions.
  • Initiate, adjust, monitor, and recommend changes in drug therapy based on patient interviews, objective monitoring parameters, and review of drug profiles and medical records.
  • Provide patient education related to disease states, including predictable actions, side effects, and reasons for proper compliance.
  • Assure ongoing performance improvement of pharmaceutical care services and provide education to providers and staff via presentations, newsletters, and one-on-one interactions.
Requirements:
  • Minimum 12 months of experience in ambulatory care pharmacy practice, including initiating, adjusting, discontinuing, and refilling medication in patients with chronic disease states.
  • Successful completion of an ASHP Accredited PGY-1 residency or equivalent experience.
  • Pharmacist License (California) and National Provider Identifier required at hire.
Preferred Qualifications:
  • Doctorate of Pharmacy, Pharm.D.
  • PGY-2 clinical pharmacy specialty residency or fellowship.
  • Board certification in relevant practice area.
