Role Summary:

We are seeking a full-time Care Coordinator I or II for our Bellingham Medical Clinic. Ideal candidates will possess either a Medical Assistant Certification or be a Licensed Practical Nurse (LPN).

The Care Coordinator plays a vital role within a clinical care team, enhancing the quality of patient-centered care. Responsibilities include:
  • Identifying gaps in care for patients with chronic conditions and/or mental health needs.
  • Collaborating with the clinical care team to develop care plans during daily huddles.
  • Assisting patients with medication management and insurance access.
  • Facilitating ongoing self-management goal setting using Motivational Interviewing techniques.
  • Maintaining accurate electronic health records to track patient adherence to care plans.
  • Organizing and leading team meetings, requiring strong organizational and communication skills.
Qualifications:
  • Certified Medical Assistant (for Care Coordinator I)
  • LPN with experience in outpatient care (for Care Coordinator II)
  • Bachelor's degree in a health-related field with at least one year of experience in community health.
  • Bilingual in English and Spanish is preferred but not mandatory.
