Job Overview

POSITION OBJECTIVE:
The Community Housing Support Specialist is responsible for delivering essential services that empower individuals to thrive within their communities. This role encompasses two primary areas of focus: Housing Transition Services and Housing Sustaining Services. The target population includes individuals facing disabilities, predominantly related to mental health or substance use challenges. These services are specifically designed for those experiencing homelessness or at risk of homelessness, as well as individuals residing in institutional settings or facing potential segregation. The ultimate goal of these services is to facilitate a smooth transition into stable housing, promote long-term community integration, and prevent future instances of homelessness or institutionalization.

Key Responsibilities:

  • Deliver community-based Housing Support Services (both transition and sustaining) to eligible adult clients (18+) with disabilities that hinder their ability to live independently.
  • Manage a caseload of clients, engaging with them 2-3 hours weekly in their homes or community settings.
  • Ensure services align with the individualized housing plan developed collaboratively with each client, emphasizing cultural sensitivity and a holistic approach.
  • Identify and address barriers to housing access.
  • Assist clients in applying for various benefits (county, state, tribal, and federal) to support their housing needs.
  • Research and connect clients with potential housing options, guiding them through the application process.
  • Educate clients on lease agreements and ensure their living environments are safe and ready for occupancy.
  • Identify resources to assist with moving costs and securing household items.
  • Support clients in maintaining their housing through proactive identification of behaviors that may threaten their stability.
  • Facilitate the housing recertification process for rental assistance and lease negotiations.
  • Provide training on tenant responsibilities, lease compliance, and effective household management.
  • Assist clients in understanding and managing their income and benefits to ensure housing retention.
  • Encourage the development of natural housing supports and resources within the community.
  • Participate in supervision and professional development under the guidance of a designated supervisor.
  • Offer transportation for clients as necessary.
  • Maintain a caseload of at least ten clients, adhering to productivity standards and actively seeking referrals.
  • Ensure compliance with billing and documentation requirements as outlined by relevant authorities.
  • Engage in client care activities, including safety evaluations and appropriate interventions.
  • Advocate for clients as needed.
  • Provide crisis intervention when necessary.
  • Attend training sessions and conferences to fulfill continuing education requirements and enhance professional knowledge.
  • Adhere to confidentiality, data privacy regulations, and organizational policies.
  • Perform additional duties as assigned.

MINIMUM REQUIRED QUALIFICATIONS:
Education:

  • High school diploma or equivalent.
License/Certification:
  • Valid driver's license, reliable transportation, and current insurance.
Knowledge, Skills, and Abilities:
  • Understanding of housing and homelessness concepts, considering mental, emotional, and physical factors.
  • Ability to apply critical thinking, work independently, and make informed decisions based on established standards.
  • Effective time management and organizational skills.
  • Basic knowledge of social work principles and practices.
  • Ability to handle sensitive situations with professionalism and communicate effectively with diverse audiences.
  • Strong observation, assessment, and crisis management capabilities.
  • Ability to manage stress and respond to challenging client behaviors.
  • Capacity to work independently and collaboratively within a team.
  • Excellent organizational and documentation skills.
  • Familiarity with Microsoft Office Suite and electronic file management.
QUALIFICATIONS:
  • Bachelor's degree in social sciences (e.g., Social Work, Psychology, Human Services, Chemical Dependency, Sociology) or equivalent experience in a community service setting.
WORK ENVIRONMENT:
This position involves working in various settings, including office environments, remote locations, and clients' homes. Employees may encounter normal office conditions with varying noise levels. Travel within the community is required to provide services.

PHYSICAL DEMANDS:
Employees may need to stand, walk, sit, bend, twist, and perform repetitive motions. The role may involve prolonged periods of sitting and driving. Employees should be able to lift and/or move up to 50 pounds as needed.

MENTAL DEMANDS:
This position requires the ability to manage multiple tasks, meet deadlines, and interact with a diverse range of individuals on complex issues.

OTHER:
All employees must maintain confidentiality and comply with privacy regulations. A criminal history background check is required for this position.
