Clinical Health Care Navigator

About the Role
At CenterWell Home Health, we are seeking a highly skilled Clinical Health Care Navigator to join our team. As a Clinical Health Care Navigator, you will play a crucial role in providing personalized home care to patients managing a chronic condition or recovering from injury, illness, surgery, or hospitalization.
Key Responsibilities
- Develop and Implement Care Plans
Create and implement comprehensive care plans that address the unique needs of each patient, ensuring optimal clinical outcomes and a smooth transition back to independence.
Coordinate Care TeamLead a multidisciplinary care team, including nurses, physical therapists, occupational therapists, speech-language pathologists, home health aides, and medical social workers, to ensure seamless coordination of care and services.
Communicate EffectivelyCommunicate regularly with patients, families, and healthcare providers to ensure timely and effective care delivery, addressing any concerns or questions they may have.
Monitor ProgressRegularly assess patient progress, identifying areas for improvement and making adjustments to care plans as needed to ensure optimal outcomes.
Requirements
- Bachelor of Science degree in Nursing (BSN) preferred
- At least 2 years of clinical home care experience with a minimum of one year in a clinical management role
- Current CPR certification
- Excellent verbal and written communication skills
- Excellent interpersonal skills
- Knowledge of state and federal home health agency regulations and compliance standards
- Salary Range
$69,800 - $96,200 per year
Approximate percent of time required to travel: 60%About Us
