About the RoleWe are seeking a skilled Advanced Care Coordinator to join our team. In this role, you will provide high-quality patient care and work closely with nursing staff, physicians, and other healthcare professionals.
Responsibilities- Provide client services to local Skilled Nursing & Rehabilitation facilities, ensuring quality care for patients.
- Assure quality nursing care by directing and coordinating nursing activities.
- Conduct physicals and annual assessments for assigned patients, identifying changes in condition and managing chronic conditions effectively.
- Complete acute visits to assess and diagnose patient conditions, making recommendations for further interventions as needed.
- Focus on early identification of changes in patient condition and strong management of chronic conditions.
- Complete history and physical on new admissions within 72 hours, ensuring timely documentation.
- Complete Medicare certifications according to required time frames, maintaining accurate records.
- Prepare discharge orders, instructions, and prescriptions for patients being discharged, promoting seamless transitions.
- Encourage advanced care planning and end-of-life decisions for patients requiring these services, providing emotional support.
- Provide pharmacy with needed narcotic prescription refills as patient need dictates, ensuring continuity of care.
- Document patient assessment and plan of care in the electronic medical record, facilitating effective communication among healthcare teams.
- Review labs and other diagnostic tests for additional interventions, making informed decisions.
- Collaborate with physicians and staff to ensure comprehensive patient care, sharing knowledge and expertise.
- Notify family members of changes in patient condition, maintaining open lines of communication.
- Communicate daily with nursing staff, assigning duties, supervising, and evaluating work performance, promoting a positive work environment.
- Review nursing policies and procedures for compliance, making recommendations for changes or additions as necessary.
- Recommend or conduct specific areas of in-service or continuing education, enhancing professional development.
- Assess patients for palliative care needs, creating and overseeing palliative plans of care in facilities and patient homes.
- Current, unrestricted valid Nurse Practitioner license from the Board of Nursing or the ability to obtain.
- DEA certificate current or the ability to obtain.
- Clinical background in adult, family, or geriatric specialties preferred.
- Past experience working in long-term care, nursing homes, or with seniors in other settings preferred.
- Strong communication and relationship-building skills preferred.
- Intermediate level of proficiency in PC-based word processing, including Microsoft Word and Outlook.
- Willingness to be 'on call' for Facilities as needed.
- Dependable, flexible, and able to work well with all nursing personnel and other departments.
- Ability to make decisions regarding nursing problems, work well under pressure, and problem-solve.
