Job Summary

The Nurse Practitioner at St. Luke's Corporation collaborates with Physicians to provide primary health care services, differentiating between normal medical findings and those requiring treatment consultation or referral. The ideal candidate evaluates and monitors health status, performs diagnostic and therapeutic services, and provides age-appropriate education and counseling for disease prevention, disease management, and health maintenance.

Responsibilities
  • Evaluate and monitor health status, performing diagnostic and therapeutic services as needed.
  • Provide age-appropriate education and counseling for disease prevention, disease management, and health maintenance.
  • Collaborate with Physicians to coordinate a continuum of care for patients.
  • Establish and maintain harmonious and cooperative working relationships with other healthcare professionals.
  • Effectively communicate with patients, family members, Physicians, and support staff.
Requirements
  • Education: Registered Nurse with additional training through an advanced studies program.
  • Experience: In an ambulatory care/primary care setting, with a minimum of 6 months of experience in stroke care.
  • Licensure/Certification/Registration: Licensed to practice in the State of Minnesota or Wisconsin, and certified as a Certified Nurse Practitioner in the specialty of adult, family, pediatric, or OB/GYN.
Preferred Qualifications
  • Family practice experience.
Knowledge, Skills, and Abilities
  • Knowledge of current practice standards for Nurse Practitioners.
  • Demonstrated skills in independent decision-making, management of minor acute illness and chronic health problems, and routine physical examinations.
  • Ability to provide age-appropriate care to patients of all ages.
  • Ability to collaboratively work with the patient's Physician in coordinating a continuum of care.
  • Ability to establish and maintain harmonious and cooperative working relationships with other people.
Physical Demands and Environment
  • Ability to meet the physical demands of primary care practice, including prolonged standing, walking, and reaching.
  • Lifting requirements: up to 10 pounds continuously, up to 25 pounds frequently, and up to 35 pounds occasionally.
  • Work environment: typical noise level, moderate noise, and exposure to risk of blood-borne diseases.
