Job Summary

The Street Medicine Peer Support Specialist will provide peer coaching and support to individuals experiencing homelessness, while working collaboratively with the Street Medicine Team to engage clients and provide follow-up services. This role requires a strong understanding of harm reduction techniques and the ability to work effectively with clients and community partners.

Key Responsibilities
  • Gain trust and respect of clients and potential clients based on shared experience, mutual support, and perspectives.
  • Model recovery and skills that are conducive to community participation and inclusion.
  • Provides street outreach in tandem with Street Medicine Team to engage clients who are defined as unsheltered.
  • Provide follow-up services to known locations as identified by the person served.
  • Provides intensive support services in tandem with Community Health Worker.
  • Performs screening and assessments with clients as needed for continuity of services.
  • Coordinates with care team for completion of case plan.
  • Mentor the client on navigating human service systems.
  • Directly engages community partners, service providers, and landlords/property managers.
  • Assist clients with obtaining necessary identity documentation.
  • Documents services provided in a timely and accurate manner, according to NSO policy.
  • Provide wraparound services to bridge gap between medical, basic needs, and housing service teams for persons experiencing unsheltered homelessness.
Requirements
  • Certified Peer Support Specialist certification.
  • Chauffeur's license.
  • High School Diploma or equivalent (GED).
  • Must have lived experience of homelessness.
  • Ability to demonstrate an understanding of harm reduction techniques as it applies to substance use preferred.
  • Experience collaborating with a medical care team preferred.
Working Conditions

This position requires extensive driving in the community and will require the transport of clients. Staff should be prepared for all weather conditions, as they will be spending a considerable amount of time outdoors and in vehicles.
