About the Job

We are seeking a highly motivated and detail-oriented LPN to join our team as a Bedside Care Coordinator. This is a fantastic opportunity to make a real difference in the lives of individuals with developmental disabilities and brain injuries. As an LPN, you will be responsible for monitoring and coordinating healthcare services, communicating effectively with healthcare professionals, and documenting all nursing interventions and changes affecting the individual.

Key Responsibilities

- Coordinate healthcare services for program participants, working closely with physicians, nurse practitioners, and other medical providers
- Develop and implement individualized care plans, incorporating input from healthcare professionals and family members
- Conduct regular assessments to identify potential health risks and develop strategies to mitigate them
- Collaborate with the multidisciplinary team to ensure seamless delivery of care services

Requirements

- High School Diploma/GED required; College degree preferred
- Valid MA Nursing License and valid driver's license with acceptable driving record
- Strong communication and interpersonal skills, with ability to work effectively with diverse stakeholders
