Key Responsibilities:

  • Coordinate healthcare services for program participants
  • Develop and implement individualized care plans
  • Provide education and training to staff on patient care and medical procedures
  • Collaborate with multidisciplinary teams to ensure comprehensive care
  • Participate in quality improvement initiatives to enhance patient outcomes

Requirements:

  • Associate's degree in nursing or higher
  • Valid LPN licensure in Massachusetts
  • Experience working with individuals with developmental disabilities and brain injuries
  • Strong communication and interpersonal skills

Compensation and Benefits:

  • Annual salary: $68,640 - $79,040 (based on experience/education)
  • Sign-on bonus: up to $5,000
