Clinical Nurse Specialist

We are seeking a skilled and compassionate Registered Nurse to join our team at Mass General Brigham Healthcare at Home. As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to individuals in their homes, promoting independence and improving quality of life.
Responsibilities- Assess patients' needs and develop individualized care plans to meet their unique requirements.
- Provide skilled nursing care, including assessment, teaching, treatment, and documentation, while demonstrating excellent customer service and financial awareness.
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
- Participate in ongoing education and training to stay up-to-date on best practices and industry developments.
- Graduate of an approved School of Nursing.
- Current Massachusetts license as Registered Nurse.
- Minimum of one year previous nursing experience in an acute care setting, with prior home care experience preferred.
- Ability to work independently, prioritize tasks, and adapt to changing situations.
- Strong interpersonal and customer service skills, with the ability to work effectively with diverse populations.
