About the Role

We are seeking an experienced Oncology Nurse Navigator 2 to join our team at Rush University Medical Center. As a key member of our cancer care team, you will play a vital role in promoting effective utilization of services and coordination of care for patients within our Cancer Center service line.

Key Responsibilities
  • Establish and maintain therapeutic relationships with patients and families, utilizing effective communication and managing a defined patient caseload.
  • Assess patients for social determinants of health and develop plans of care and mutual goal setting.
  • Collaborate with healthcare teams to ensure coordination of patient care and advocate for patients and their families to ensure timely access to treatment and equal access to healthcare.
  • Provide psycho-social support and facilitate referrals and transitions of care, ensuring effective utilization of internal and community resources.
  • Perform role utilizing evidence-based practice within specialty established by nationally recognized oncology nursing associations, cancer societies, and networks.
Requirements
  • Current State of Illinois Registered Nurse licensure required.
  • At least one year of nursing experience.
  • Earned nursing degree of baccalaureate level of education or higher.
  • Oncology nursing experience preferred.
  • Oncology nursing certification preferred.
