Job Summary:
  • Provide case management services to patients, families, and caregivers to promote optimal health outcomes and efficient care transitions.
  • Collaborate with healthcare teams to develop and implement individualized care plans, ensuring that patients receive the necessary resources and support.
  • Conduct thorough assessments of patients' needs, identifying opportunities for improvement and developing strategies to address them.
  • Develop and maintain relationships with community resources, healthcare providers, and other stakeholders to ensure seamless care coordination.
  • Monitor and evaluate the effectiveness of care plans, making adjustments as needed to ensure optimal outcomes.

Requirements:
