Job Title: Registered Nurse - Case Management

Tenet Massachusetts is seeking a skilled Registered Nurse to join our team as a Case Manager. This is a full-time position that requires a strong background in nursing and case management.

Job Summary:

The RN Case Manager will be responsible for coordinating patient care across the continuum, ensuring that patients receive the right care at the right time. This includes assessing patient needs, developing care plans, and coordinating with healthcare providers to ensure seamless transitions.

Responsibilities:
  • Assess patient needs and develop individualized care plans
  • Coordinate with healthcare providers to ensure timely and effective care
  • Monitor patient progress and adjust care plans as needed
  • Collaborate with interdisciplinary teams to ensure comprehensive care
  • Ensure compliance with regulatory requirements and hospital policies
Requirements:
  • Current RN licensure in the State of Massachusetts
  • 2 years of RN acute care hospital experience
  • Acute care case management experience preferred
  • Accredited Case Manager (ACM) certification preferred
