Job Summary

We are seeking a compassionate Nurse Practitioner to provide evidence-based medical care to seniors and patients with complex needs.

Abode Care Partners is a leading provider of integrated medical services caring for individuals from post hospitalization to home in various settings ranging from skilled nursing facilities, assisted living, independent living, group homes, and private homes.

Key Responsibilities
  • Provide Comprehensive Medical Care: Examine and treat chronic illnesses, order diagnostic testing, and collaborate with healthcare professionals to maintain comprehensive care plans.
  • Maintain Accurate Patient Records: Ensure patient medical records are kept up to date.
Required Skills and Qualifications
  • Licensure and Certification: Active state Nurse Practitioners license, current Nurse Practitioner National Board Certification, and active state DEA licensure required.
  • Education: Master's degree in Nursing (MSN).
  • Experience: Five or more years of nursing experience; at least two years of nurse practitioner experience preferred.
  • Communication and Interpersonal Skills: Strong interpersonal communication skills – verbal and written.
  • Time Management: Excellent time management skills with accountability for practitioner's daily workflow.
  • Technical Skills: Proficiency in medical instruments and equipment, electronic medical record (EMR) systems, and Microsoft Office applications.
