About the Role

We are seeking a compassionate and skilled Medical Care Specialist to join our team at Abode Care Partners. As a Nurse Practitioner, you will provide high-quality medical care to seniors and patients with complex needs in various living environments.

Key Responsibilities
  • Perform comprehensive evaluations to diagnose and treat acute and chronic illnesses
  • Collaborate with a multidisciplinary team to enhance patient care and develop personalized treatment plans
  • Order and interpret laboratory and diagnostic testing as needed
  • Provide referrals to other healthcare practitioners when necessary
Requirements
  • Active state Nurse Practitioners license
  • Current Nurse Practitioner National Board Certification
  • Active state DEA licensure
  • Master's degree in Nursing (MSN)
  • Five or more years of nursing experience; at least two years of nurse practitioner experience preferred
