About the Role

We are seeking a skilled Nurse Practitioner (APRN) to join our team. As a key member of our interdisciplinary care team, you will provide compassionate, evidence-based medical care to patients in their homes, skilled nursing facilities, assisted living communities, or other settings.

Your responsibilities will include conducting comprehensive evaluations, treating acute and chronic illnesses, and collaborating with caregivers and other healthcare professionals to enhance patient care and outcomes.

In this role, you will have the opportunity to work closely with our experienced team to develop and implement individualized care plans that address the unique needs of each patient.

What You'll Need
  • A Master's degree in Nursing (MSN)
  • An active state Nurse Practitioners license
  • Current Nurse Practitioner National Board Certification
  • Active state DEA licensure
  • Five or more years of nursing experience; at least two years of nurse practitioner experience preferred
