Responsibilities- Conduct psychological screenings and develop individualized plans of care to support resident adjustment to rehabilitation or long-term care.
- Collaborate with residents, family members, and healthcare teams to identify goals and develop strategies for achieving them.
- Ensure accurate and timely completion of all necessary paperwork, including the Minimum Data Set (MDS).
- Provide education and support to residents and their families on various topics, including grief and loss, acceptance, and coping with changes in living arrangements.
- Coordinate discharge services, including planning and arranging for follow-up care.
- 1-3 years of experience in long-term care or a related field.
- Bachelor's degree in Social Work, Psychology, or a related field.
- Current Social Work licensure.
