About Us

PATH is committed to ending homelessness in California by providing essential services, advocacy, and support to people experiencing homelessness. Our team of dedicated professionals works tirelessly to address the root causes of homelessness and provide solutions that lead to lasting change.

Estimated Salary Range

$23.24 - $28.28 per hour, depending on experience.

Job Responsibilities

  1. Provide case management services, including independent living skills, housing stabilization, money management, community integration, employment linkage, benefits establishment, community integration, and linkage to community providers for substance use, primary, and mental health care;
  2. Develop individualized treatment plans that address short- and long-term goals;
  3. Maintain documentation standards as set forth by the program contract and PATH policies;
  4. Collaborate with clients to assess plan progress and ensure goals are met or adjusted as necessary;
  5. Provide strengths-based case management and service coordination designed to assist clients in obtaining and maintaining stable housing.

Requirements

  • High school diploma or equivalent required; 2 years of experience in healthcare, case management, homeless services, or working with vulnerable populations preferred;
  • Demonstrated knowledge of evidence-based case management techniques, including critical time intervention, trauma-informed care, harm reduction strategies, crisis intervention techniques, and motivational interviewing;
  • Ability to maintain confidentiality and handle sensitive information according to HIPAA standards;
  • Possess a high level of tolerance and understanding for individuals with urgent and multiple case management and health needs;
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.

CALIFORNIA-BASED WORK EXPECTATIONS

We are a California employer. Therefore, all regular and customary work must be performed within the state. Approval for work outside of the office, does not establish approval for work to be conducted outside of California.
