Position Overview


The Registered Nurse Care Coordinator is accountable for overseeing all facets of patient care in alignment with a defined interdisciplinary care strategy. This role necessitates collaboration with various healthcare professionals to strategize, monitor, and assess patient care outcomes.

Key Responsibilities


The responsibilities of the Registered Nurse Care Coordinator encompass, but are not limited to, the following: * Conducts comprehensive nursing evaluations and executes patient-specific interventions as outlined in the care plan * Oversees and coordinates all elements of home health services assigned to patients and secures necessary authorizations for treatment prior to service delivery * Manages and reviews all incoming laboratory results, promptly notifying the relevant physician and providing follow-up as needed * Processes new physician directives and updates the appropriate members of the patient care team regarding these orders * Conducts admissions, assessments, recertifications, discharges, and self-scheduling for all RN visits * Ensures timely maintenance and submission of all required documentation in accordance with agency and home health policies and procedures * Availability for rotating on-call duties is required * Supervises LPNs and Home Health Aides delivering patient care to enhance quality outcomes * Engages in case conferences and recertification processes, ensuring the implementation of activities aimed at improving patient care quality as mandated by the agency * Participates in the orientation and skill development of new staff as assigned and supervises and educates students during their clinical rotations The Registered Nurse Care Coordinator reports to the Clinical Manager.

Qualifications

  • Associate's or Bachelor's degree in Nursing
  • Current licensure as a Registered Nurse (RN) in the State of Oklahoma or a current multistate license from a Nurse Licensure Compact (eNLC) member state
  • Current CPR certification upon hire, or completion within 30 days of hire; 2 years of RN experience required; 2 years of LPN experience can be substituted for one year of RN experience; 2 years of home health experience preferred
  • Valid Oklahoma driver's license and verification of automobile insurance
  • Must possess effective communication skills in English (both verbal and written)
  • This role requires incumbents to operate an INTEGRIS-owned vehicle or a personal vehicle (non-INTEGRIS-owned) and therefore must have a current Oklahoma State Driver's License along with a driving record acceptable to our insurance provider.
