About the Role

Guild is seeking a skilled Case Manager to join our team in providing mobile case management services to individuals experiencing homelessness. As a Case Manager, you will establish therapeutic engagement with individuals served, assess their strengths and areas for growth, and develop treatment goals to support their recovery.

Key Responsibilities
  • Establish rapport with a diverse population and provide interventions to determine their needs and goals.
  • Develop and implement housing supports for individuals with disabilities and those experiencing homelessness.
  • Connect individuals with community and healthcare resources to achieve housing retention.
  • Utilize evidence-based practices such as Housing First, Motivational Interviewing, and Harm Reduction to support recovery.
  • Facilitate services with individual's community treatment team and provide crisis intervention practices.
  • Document services in a timely and accurate manner.
Requirements
  • Bachelor's Degree in a behavioral science or related field from an accredited college or university.
  • At least 2,000 hours (1 year) of experience in delivering services to individuals with mental illness.
  • OR qualified as a mental health practitioner within the state of Minnesota by one of the following: at least 30 semester/45 quarter credits and 2,000 hours of supervised experience, or 4,000 hours of supervised experience.
Preferred Qualifications
  • Experience providing Targeted Case Management services.
  • Experience providing homeless outreach services and/or serving people experiencing homelessness.
  • Knowledge of income, benefits, and supports available to individuals served through federal, state, and county programs.
