We are seeking a highly skilled and experienced Pharmacy Clinical Specialist to join our team at Houston Methodist Cypress Hospital. As a key member of our pharmacy team, you will be responsible for providing expert clinical pharmacy services to patients and healthcare professionals.
Key Responsibilities- Provide expert clinical pharmacy services to patients and healthcare professionals, including medication therapy management and patient education.
- Develop and implement medication therapy protocols and guidelines to improve patient outcomes.
- Collaborate with healthcare teams to ensure safe and effective medication use.
- Conduct medication use evaluations and quality improvement initiatives to optimize patient care.
- Precept and mentor pharmacy students and residents to ensure the development of future clinical pharmacy leaders.
- Pharmacist licensure in the state of Texas or eligibility for licensure within 90 days.
- Completion of an ASHP-accredited postgraduate year 1 (PGY-1) and year 2 (PGY-2) residency training program or equivalent professional experience.
- Three years of clinical practice experience or equivalent experience as a Clinical Specialist.
- Strong communication and interpersonal skills, with the ability to work effectively with healthcare teams.
- Ability to analyze complex medication-related problems and develop effective solutions.
