Job Description
This Registered Nurse (RN) Admissions position at Crossroads Hospice & Palliative Care involves providing high-quality end-of-life care to patients and their families. The ideal candidate will possess strong clinical skills, excellent communication abilities, and a compassionate approach to care.

Salary and Schedule
This role offers a competitive salary of $87,500 per year, paid bi-weekly, in exchange for a Monday to Friday schedule from 11:00 am to 7:30 pm. No mandatory on-call shifts are required.

Key Responsibilities
As a member of our interdisciplinary team, you will be responsible for:

  • Providing direct patient care, assessing and monitoring patients' conditions, administering medications, and performing treatments
  • Collaborating with healthcare team members to develop and implement individualized care plans
  • Conducting ongoing assessments to evaluate the effectiveness of the care plan and making necessary adjustments

Requirements
To succeed in this role, you will need:

  • 1+ years of prior skilled nursing experience
  • An active, unencumbered state RN license
  • Current driver's license and auto liability coverage, as well as reliable personal transportation
  • CPR certification, a copy of TB results, and an updated physical
  • Hospice or Home Health experience is preferred but not required
