We are seeking a Licensed Nursing Assistant to join our team. The ideal candidate will have a valid State of Vermont Nursing Assistant License and a High School Diploma or GED.

Responsibilities include:

* Under the supervision of a Registered Nurse, receives daily patient assignments and carries out assignments in accordance with individualized patient plan of care.

* Collects patient and family data and reports findings to RN in a timely manner.

* Assists in the communication of information to assigned patients.

* Performs a variety of patient care tasks including, but not limited to activities of daily living, collection of vital signs, ambulation/exercises, etc.

* Serves as Hospice Aide as directed.

* Serves as Personal Care Attendant as directed.

Requirements:

* Valid State of Vermont Nursing Assistant License

* High School Diploma or GED

* Valid Driver's License and Current Auto Insurance

* Experienced or newly graduated LNA's welcome

* Must be at least 18 years of age

* Weekend and holiday rotation required
