Job Summary

Your primary responsibilities will include evaluating physician medication orders, maintaining patient databases, and collaborating with healthcare teams to ensure safe and effective medication management.

In this role, you will have the opportunity to grow your pharmaceutical skills and contribute to the success of our organization.

Key Responsibilities:

  • Evaluate physician medication orders for appropriateness and make recommendations for therapy changes as needed
  • Maintain accurate and up-to-date patient databases, including current and past medical history, clinical and laboratory data, and concurrent medication
  • Collaborate with healthcare teams to provide clinical and technical support, including ordering and evaluating laboratory tests
  • Document all communication with physicians, nurses, and other healthcare team members in the medical record
  • Implement policies and procedures for safe medication procurement, storage, distribution, and disposal

Requirements:

  • Bachelor's Degree in Pharmacy; Pharm D preferred
  • One year experience in long-term care
  • Detail-oriented, goal-oriented, and team player with excellent oral communication skills
