Hamilton Madison House is a non-profit organization dedicated to providing supportive services to seniors living in the community. Our mission is to promote healthy aging, independence, and quality of life for our clients.

We are seeking a highly motivated and experienced care manager specialist to join our team. The successful candidate will be responsible for providing individualized case management services to seniors, including conducting intakes, assessments, and developing plans of care.

About the Role:

This is a full-time position working Monday through Friday, 9:00AM to 5:00PM, with 35 hours per week. The ideal candidate will have a Master's degree in social work or a related field and at least 2 years of experience working with older adults.

The successful candidate will possess strong communication and interpersonal skills, with the ability to work effectively with diverse populations. They will also have excellent organizational and time management skills, with the ability to prioritize tasks and manage multiple projects simultaneously.

Responsibilities:
  • Provide individualized case management services to seniors, including conducting intakes, assessments, and developing plans of care.
  • Work collaboratively with other members of the interdisciplinary team to ensure comprehensive care coordination.
  • Develop and implement group activities, educational programs, and health promotion initiatives to support the needs of seniors.
  • Maintain accurate records and documentation, ensuring compliance with regulatory requirements.
  • Participate in outreach activities and develop partnerships with community organizations to support the mission of Hamilton Madison House.
