Key Responsibilities:
  • Cover all areas such as housing, physical and mental health, education, food security, dental, and activities of daily living
  • Ensure timely completion of tasks and responsibilities
  • Maintain accurate and up-to-date documentation of patient information
Required Skills and Qualifications:
  • Bachelor's degree in a related field (e.g., nursing, social work, healthcare administration)
  • 2+ years of experience in healthcare case management or a related field
