At Amida Care, we're committed to delivering innovative and effective care models that prioritize member health outcomes. As a High Risk Care Specialist, you'll play a critical role in our Integrated Care Team, responsible for the intensive care management of high-risk members. This position requires strong clinical oversight skills, as well as the ability to develop person-centered care plans and coordinate services to meet complex medical, behavioral, pharmaceutical, and psychosocial needs.

Key Responsibilities:

  • Provide single-point coordination and accountability in managing the care of high-risk members
  • Develop and implement comprehensive care plans based on thorough needs assessments
  • Collaborate with treatment providers and community agencies to assess, plan, and implement care plans
  • Contact the Medical Team and Discharge Planning staff for members admitted to inpatient settings to improve health outcomes
  • Ccoordinate member access to services such as home health care, Medical Home, durable medical equipment, and hospice care
  • Offer education on topics like Plan benefits, authorization processes, cancer screenings/prevention, and health promotion

Requirements:

  • NYS Licensed Practical Nurse experience required; three years of managed care experience preferred
  • Experience working with people living with HIV/AIDS or serious chronic illness preferred
  • Strong knowledge of Microsoft Office and willingness to learn other platforms as needed
  • Demonstrate understanding and sensitivity to multicultural values, beliefs, and attitudes

Salary Range:$61,117 - $76,418 per year
