POSITION SUMMARY
The Senior Field Manager role at Adobe Care And Wellness LLC is pivotal in overseeing the operations of our community health workers, known as Care Navigators. This position is essential for ensuring that our team effectively contributes to patient care and the overall efficiency of our healthcare services. Responsibilities include coordinating schedules, managing patient interactions, and ensuring adherence to healthcare regulations.
In this capacity, you will lead the ongoing training and development of your team, fostering a culture of continuous improvement and exemplifying best practices. A significant aspect of this role involves traveling to support Care Navigators in the field, providing guidance and reviewing processes to enhance performance and efficiency.
The primary function of Care Navigators is to conduct in-home assessments and facilitate access to necessary resources, ensuring patients remain healthy and satisfied. As the Senior Manager of Clinical Operations, you will serve as the main point of contact for field teams, acting as a subject matter expert on quality and operational initiatives that impact their work. This includes conducting audits, reviewing patient charts, and ensuring that patient care and quality outcomes remain a top priority.
This newly established position is crucial for supporting our growth and reports directly to the Senior Director of Quality.
KEY RESPONSIBILITIES
- Achieve departmental goals by managing staff, planning activities, and meeting performance indicators.
- Develop and enforce policies, procedures, and productivity standards.
- Supervise a team of Care Navigators in collaboration with Field Managers and Regional Leads.
- Oversee recruitment, orientation, and training of new employees.
- Provide coaching, performance evaluations, and set clear job expectations.
- Continuously assess the competency of the Care Navigator team and provide necessary education and mentorship.
- Build strong relationships with clients, caregivers, and clinical leadership.
- Travel as needed for training and oversight, which may include overnight stays.
- Conduct in-home assessments and educate staff on clinical documentation and health conditions.
- Participate in quality assurance initiatives and represent the team in quality committees.
- Audit patient charts for accuracy and compliance with regulations.
- Identify documentation errors and implement corrective actions.
- Collaborate with quality assurance teams to refine audit processes.
- Design and deliver training on documentation and compliance standards.
- Provide ongoing education to Care Navigators on documentation importance.
- Conduct field visits to support Care Navigators and ensure adherence to standards.
- Maintain compliance with HIPAA regulations and protect patient information.
- Commit to personal and professional growth through continuous education and training.
QUALIFICATIONS
- Minimum of three years of management experience.
- Minimum of three years of field experience in healthcare.
- Strong leadership and remote management skills across multiple locations.
- Excellent written and verbal communication skills.
- Detail-oriented with strong problem-solving abilities.
- Ability to work flexible hours as required.
- Proficient in building relationships and fostering teamwork.
- Experience with Medicare Advantage, Medicaid, and HEDIS quality measures.
- Familiarity with electronic medical records and compliance documentation.
- Proficient in Microsoft 365 and other relevant technologies.
EDUCATION & CERTIFICATIONS
- Associate degree preferred.
- Certified Medical Assistant certification required.
- Basic Life Support (BLS) certification required.
- Fingerprint clearance card required.
