Family Support Specialist

At Health Connect America, we are dedicated to providing high-quality, compassionate care to children and families in need. As a Family Support Specialist, you will play a critical role in delivering best practice clinical services, ensuring accurate documentation, and collaborating with our treatment team to develop effective treatment plans.

Responsibilities:
  • Provide individual, group, and family therapeutic interventions and skill building as outlined in the treatment plan
  • Implement Health Connect America approved therapeutic interventions with clients and their families
  • Complete all client documentation within established time frames
  • Attend and actively participate in weekly Treatment Team meetings
  • Link clients and families with specific services and resources as identified in the treatment plan
  • Serve as a liaison with other professionals and agencies as related to the client
  • Actively recruit new referrals to ensure a full caseload
  • Provide 24/7 on-call support for clients as per program requirements
  • Ensure compliance with all state regulatory bodies and COA standards
Qualifications:
  • Bachelor's degree in a human services discipline, including Social Work, Psychology, Sociology, Counseling, or Criminal Justice
  • 1 year of experience working with children, or BA/BS degree with 3 years of experience working with children
