Key Responsibilities- Provide comprehensive patient care, including assessment, diagnosis, and treatment planning, in a fast-paced ambulatory care setting.
- Collaborate with healthcare teams to develop and implement patient-centered care plans, ensuring seamless transitions and optimal patient outcomes.
- Analyze unit-level quality data and participate in quality improvement initiatives to drive excellence in patient care and safety.
- Contribute to the development and implementation of clinical practice guidelines and standards, ensuring evidence-based care and best practices.
- Engage in self-assessment and peer review, utilizing resources to meet individual learning needs and promote professional development in clinical practice.
- Serve as a preceptor and mentor for nursing staff and students, fostering a culture of excellence and knowledge sharing.
- Organize staff assignments to ensure efficient delegation of activities, promoting a safe and effective work environment.
- Comply with regulatory requirements, professional standards, and organizational policies, ensuring adherence to best practices and industry standards.
- Analyze and contribute to unit throughput efficiencies and metrics, driving continuous improvement and excellence in patient care.
- Coordinate and direct unit activities to provide patient care in a healing, humane, and caring environment, promoting a positive patient experience.
- Evaluate patient progress, involving patients and families in the evaluation process, and responding to variations in patient progress by modifying interventions as needed.
- Educate patients and families regarding health status and established plans of care, determining learning needs and implementing educational plans to promote patient empowerment and self-management.
- Direct the coordination of patient care in a safe, timely, and appropriate manner, applying and supporting the escalation process, and considering the needs and behaviors of specific patient age groups and cultural groups in all patient care activities.
- Collect, assess, and document patient healthcare data, directing patients and families to identify realistic and measurable plans of care, prioritizing care based on collected data, and documenting assessment and reassessment in medical records.
- Registered Professional Nurse in New York with a Baccalaureate degree in Nursing, required.
- Fluent written and spoken English language skills.
- Required licenses: Registered Nurse License-NYS.
- Preferred qualifications: Membership in pertinent professional organizations.
