Ambulatory Care Nurse

NYU Grossman School of Medicine is seeking a highly skilled and compassionate Ambulatory Care Nurse to join our team. As an Ambulatory Care Nurse, you will play a vital role in providing high-quality patient care in an outpatient setting.
Job Summary:The Ambulatory Care Nurse will be responsible for coordinating, integrating, and communicating patient care services in a fast-paced outpatient environment. You will work closely with a multidisciplinary team to develop and implement patient-centered plans of care, ensuring seamless transitions and optimal patient outcomes.
Key Responsibilities:- Assess and prioritize patient needs, developing and implementing individualized care plans
- Collaborate with healthcare providers to coordinate patient care, including medication management and treatment plans
- Provide education and support to patients and families, promoting health literacy and self-management
- Participate in quality improvement initiatives, contributing to the development and implementation of evidence-based practices
- Communicate effectively with patients, families, and healthcare teams, ensuring clear and timely information exchange
- Baccalaureate degree in Nursing (or Diploma or Associates Degree in Nursing with plans to earn a baccalaureate degree)
- Current Registered Professional Nurse license in New York State
- Basic Life Support Certification through the American Heart Association
- Fluent written and spoken English language skills
- Advanced Cardiovascular Life Support (ACLS) certification
- Membership in pertinent professional organizations
