Our clinics utilize a collaborative care model where our physicians lead a multidisciplinary team dedicated to supporting the physical, emotional, and social well-being of our patients.

Key Responsibilities:
  • Assess and treat patients in alignment with established care standards.
  • Monitor patient care quality and adherence to medical protocols through data analysis and chart reviews.
  • Facilitate coordination of patient services, including specialty referrals and home health care.
  • Actively engage with the care team, providing medical expertise during daily meetings.
  • Collaborate with the regional medical director and center administrator to foster a cooperative practice environment.
  • Ensure timely completion of medical documentation, working with quality coders to enhance coding accuracy.
  • Adhere to policies and protocols set forth by Clinical Leadership.
  • Participate in discussions regarding quality of care and review outcome data.
  • Contribute to potential growth opportunities for new or existing services.
  • Engage in the local primary care on-call program as necessary.
  • Maintain compliance with all licensing and accreditation requirements.
  • Dedicate 100% of your time to direct patient care, including patient-facing activities and related administrative tasks.

Required Qualifications:
  • Current and unrestricted medical license or willingness to obtain necessary licenses.
  • Graduate of an accredited MD or DO program.
  • Exceptional verbal and written communication abilities.
  • Strong interpersonal skills and the ability to build relationships with colleagues and patients.
  • Commitment to the integrated team-based care model.
  • Willingness to adapt to a value-based care environment.
  • Excellent patient service skills.
  • Basic proficiency in computer usage, including email and electronic medical records.
  • Compliance with TB screening requirements for patient-facing roles.

Preferred Qualifications:
  • Board certification or eligibility in Family Medicine, Internal Medicine, or Geriatric Medicine.
  • Active DEA license.
  • Medicare Provider Number.
  • Two to five years of relevant experience.
  • Experience managing a Medicare Advantage patient panel.
  • Familiarity with Medicare guidelines and HEDIS quality indicators.
